DE PORTOLA MIDDLE SCHOOL
2021 – 2022 ORIENTATION
AUGUST 26TH, 2021
3:00PM – 4:00PM – 6th Graders
3:30PM – 4:30PM – 7th & 8th Graders
FORMS TO BE COMPLETED AND RETURNED:
K-12 ENROLLMENT FORM (BLUE PAPER): Verify your child’s information for any changes. (YOU MUST COMPLETE THIS FORM ON PARENT PORTAL) If you need your parent portal login information, please contact Earl Hamilton at [email protected] .
The remaining forms will be returned during Orientation. We will have multiple stations set up. Please follow in numerical order.
STATION 1
UNIVERSAL FORM (WHITE PAPER): The Universal Form must be completed by checking the appropriate boxes, signed by the parent and student. This form is needed for the use of student computer and media release. You may refer to The Facts for Parents on our website.
STATION 2
DE PORTOLA FOUNDATION table, become a Panther Parent! See the Foundation letter in this packet for more information. Drop donations here.
STATION 3
PE UNIFORM PURCHASE REQUEST (YELLOW): If you are new or need to purchase PE uniforms, please complete form and bring back during orientation. Please bring EXACT CASH or check made to De Portola Middle School.
STATION 4
HEALTH INFORMATION EXCHANGE CONSENT FORM (GOLDENROD PAPER): Complete, sign and return.
STATION 5
CLASS SCHEDULE PICK UP. YOU MUST COMPLETE, SIGN AND TURN IN YOUR UNIVERSAL FORM (STATION 1) AND HEALTH FORM (STATION 4) in order to collect schedule.
Please note if you are unable to attend orientation, you may return during school hours. We will have PE uniforms for sale before and after school as well.
